The
Dolgoff Plan Corporation will be the administrator
on all Dolgoff Plans. The administration fees will
be billed directly to the client participant's
corporation.

fees will be offered as

The
Dolgoff Plan Corporation will be informed and have
access to all information pertaining to the
following:
- All
information of the corporation necessary for
administration.
- All
information of the insurance policy of each
participant.
- The
Dolgoff Plan Corporation will be named as a
third party on all brokerage account forms for
the corporate investments.
The
above mentioned information is necessary and will
be used for perform our administration obligations
only.
We will have a separate e-mail and telephone number
for the clients to contact us pertaining to the
administration of the programs.
The client will receive all pertinent information
and forms necessary for all administrative purposes
including a calendar year end summary of the
program that will include all participants.
